• I understand the ClinCheck that has been shown to me and that actual clinical results may vary slightly and may need refinement aligners.
• I understand that I may need to have attachments on my teeth and they have been explained to me.
• I understand that I may have to have IPR (Interproximal Reduction) and this has been explained to me.
• I understand that I will need to wear a retainer at the end of treatment.
